Applications are accepted year-round from families who meet the eligibility guidelines. Once an application is completed, the EHS Family Service Worker will help you choose a contracted provider. The second phase of the application process will then be to schedule an orientation, after which your child will be fully enrolled. A child can enter the Early Head Start program at any time of the year. If sites are full, we will add your child’s name to a waiting list.
To start the enrollment process, simply contact Early Head Start (616-453-4145 ext. 2288) and schedule an appointment. During the appointment, the Family Service Worker will fill out the necessary paperwork with you. Please bring the following information with you:
-
Proof of income (last year's W-2, CDC, TANF)
-
Birth Certificate for each child to be enrolled
-
Current immunization records
-
Current medical and dental examination report and current physician's contact information
-
Medical insurance or Medicaid card
